Mockle Pox Clade II was removed from the HCID list on 1 March 2023 following reassessment of transmissibility and clinical severity. Clade I retains HCID status. Apply Level 3 PPE where clade is unknown.
Mockle Pox
Mocklepox virus (MKPV), formerly classified as a single entityMockle Pox is a viral zoonosis caused by Mocklepox virus, a member of the Orthopoxvirus genus. It causes fever, lymphadenopathy and a characteristic vesicular rash. Two clades are now recognised with differing severity and HCID status.
PPE level
Level 3 PPE
Hand hygiene using the 3Rs. Contact HART before patient contact.
Clade II (confirmed): Level 2 PPE is sufficient. If clade is unknown or unconfirmed, treat as Clade I and apply Level 3 PPE.
Patient advisory
The clade may not be known at the time of clinical contact. If in doubt, apply the higher level of PPE. Lesion fluid, respiratory secretions and contaminated materials are all infectious. Avoid touching lesions.

Risk to clinicians
Moderate to high depending on clade and level of contact. Transmission requires close physical contact, contact with lesion fluid, or prolonged respiratory exposure. Aerosol risk is lower than for classic smallpox.
